Support Coordination

Support Coordination provides service coordination for people we support in accordance to their NDIS (National Disability Insurance Scheme) funded plan, assisting them to connect with supports, build individuals’ capacity and resilience. The people we support have experienced trauma, have multiple complex needs and have been diagnosed with a disability and /or mental disorder. The goal is to enable and promote independence, development and confidence of the individuals by designing and developing a case plan and working with relevant services.
A support coordinator will support participants to understand and implement the funded supports in their plan and link them to community, mainstream and other government services. Support Coordination focuses on supporting participants to build skills and direct their life as well as connecting them to providers.

The role of a support coordinator is to:

  • Assist participants to negotiate with providers about what they will offer and how much it will cost the
  • Ensure service agreements and service bookings are
  • Help build the participant’s ability to exercise choice and control, to coordinate supports and access their local
  • Assist participants in planning ahead and to prepare for their plan
  • Assist participants to implement their plan and to also ‘optimize’ their plan ensuring that they are getting the most out of their funded supports.
  • Strengthen and enhance a participant’s ability to coordinate supports and access the community
  • Ensure service providers are meeting their obligations.

A Support Coordinator and a Specialist Support Coordinator are very similar however there are some key differences.

  • A Specialist Support Coordinator will be funded where there are additional high or complex needs in a participant’s situation and will be a qualified and experienced practitioner such as an Occupational Therapist, Psychologist or Social Worker.
  • Specialist support coordinators will support participants to manage challenges in their support environment which may include health, education, or justice services. Specialist support coordination aims to reduce barriers to implementing or using the NDIS plan.

There are 3 levels of Support coordination in NDIS fundings

Level 1 - Support Connection

 May provide some assistance in starting a participant’s Plan, connect participants with informal and community supports and other funded support (providers), and monitor Plan progress enabling an individual to achieve goals. Provide Assist Life Stages and Transitions services to prepare participants for the challenges they may face when undergoing life stage transitions, and help them to overcome any barriers

Level 2 - Support Coordination

Further assists the client to build skills needed to understand, implement and use their plan, and address any issues or barriers that may arise regarding the delivery of supports in accordance with the plan. Works with the client ensuring a mix of supports are being used to increase individual capacity in maintaining relationships, managing service delivery and living more independently in the community.

Level 3 - Specialist Support Coordination

Provides the same supports as level 2, but with the requirement for a specialised framework necessitated by specific high-level risks/needs. Specialist Support Coordinator will assist in managing challenges within the participants support environment and ensure delivery of service.
